ePACT Requirements

Additional participant information and medically verified immunization form will be required in ePACT prior to participation. New participants will receive an email the evening after completing registration with a link to their ePACT accounts. If you have a child with medical or special needs, additional paperwork is required and available for download through ePACT. For children with special needs, we may request you meet with our staff, at least two weeks prior to the start of program, to ensure we are best prepared to support your child’s success.
